What does the AVPU scale assess?

Pain, Pressure, Pulse, and Temperature.

Activity, Vision, Perception, Understanding.

Airway, Ventilation, Perfusion, and Oxygenation.

Level of consciousness: Alert, responds to Voice, responds to Pain, or Unresponsive.

Level of consciousness is what the AVPU scale evaluates. It’s a quick way to judge how awake a patient is and how they respond to stimuli on scene. Start by determining if the person is Alert—fully awake, oriented, and following commands. If not fully alert, check whether they respond to Voice—responds to spoken questions or commands. If there’s no response to voice, test for a response to Pain—apply a painful stimulus and look for purposeful movement or vocalization. If there’s no response to voice or pain, the patient is Unresponsive. This scale is favored in the field because it’s fast and repeatable, allowing you to monitor changes rapidly. Other options describe different domains or vital signs and don’t assess consciousness, so they don’t fit what AVPU measures.
